Sherif Mohamed Tumour necrosis factor-alpha and weight loss in chronic obstructive pulmonary diseases

Master's Thesis from the year 2001 in the subject Medicine - Neoplasms, Oncology, , language: English, abstract: Weight loss is a common feature in patients with chronic obstructive pulmonary disease (COPD). The clinical importance of weight loss; particularely loss of fat-free mass (FFM) has been demonstrated in its adverse effects on physical performance and quality of life. Moreover; weight loss and a low body weight are unfavorable prognostic factors in survival, independent of lung function.Mechanisms of malnutrition in those patients are not fully understood. Several factors have been implicated. Increased resting energy expenditure (REE) contributes the main hypothesis for weight loss in COPD patients. However, not all patients with COPD who lose weight are hypermetabolic.Recent data have shown that a systemic inflammatory response is present in patients with COPD. A clear evidence for a relationship between weight loss and plasma tumour necrosis factor-alpha (TNF-*) has been shown in COPD patients. TNF-* produces a cachexia-like syndrome in animal models and has been implicated as a mediator of cachexia in several clinical conditions including cancer, chronic heart failure, cystic fibrosis and anorexia nervosa.Nutritional assessment for COPD patients is essential; to identify those individuals who will benifit from nutritional support therapies and to determine baseline values to measure the effectiveness of nutritional intervention. It includes several methods, n...

From Chapter I: The history of the Reformation, especially in England, is the history of one of the most complex movements in in the history of Europe. The great changes of the sixteenth century were the results of many forces, closely, no doubt, interrelated, but yet diverse in their character. And yet it is but a superficial judgment which fails to perceive that below all the complex forces of the Reformation there lay one great force, -- the force of the revival of religion, a passion which possessed men for the recovery of a more spiritual, and therefore a more free and spontaneous, religious life. The characters of the men who played the greater parts in this movement are often as perplexing as the movement itself.

A total of 48 patients with head and neck cancer were studied. All patients were treated with radiotherapy, chemotherapy and surgery was added in some of the cases. Patients were randomized into control and the treatment arms. Three oral swabs for candidiasis was taken from all the patients, first one was taken before start of radiotherapy, second one during third week of treatment and last swab was collected at the end of treatment. Patients in the treatment arm were given prophylactic flucanozle 50 mg oral daily during the radiation treatment. The main end points of the study was to evaluate the effect of fluconazole on prevention of oral candidiasis and to determine whether fluconazole prophylaxis will reduce the severity of oral mucositis. Patients were assessed for mucositis weekly. Patients in the treatment arm had less incidence of oral candidiasis and less severe mucositis as compared to the control arm. Prophylactic use of oral flucanazole is effective in reducing the incidence of oral candidasis and also the severity of oral mucositis.

The aim of this study was to determine the preponderance of low vision and blindness in retinitis pigmentosa in patients of Okara. The techniques used for the diagnosis of retinitis pigmentosa were an examination of ocular fundus using a biomicroscope (fundoscopy), far visual acuity and visual field. Indeed, a drop in visual acuity and blindness were the main reasons for consultation in this study. 100 cases were reported of which two were with family history from Okara, 70 male and 30 females were affected from RP. The average age of RP patients was 27.45 with a range of 4-78 years. Blindness and low vision was observed in 54% of the patients and 40% respectively. The average age of RP patients with low vision ranges from 6-50 years and the average age of RP patients with blindness range from 9-65 years. Patients should be screened to enable them to benefit from management focusing on both appropriate treatment and genetic counseling.

Women of Vision is a book that expands significantly public knowledge of the contributions of Catholic laywomen to church and society over the past century.Despite historic advances in women’s recognition and equality over the past half-century, the significant roles played by Roman Catholic laywomen in church and society still go largely unacknowledged. With Women of Vision: Sixteen Founders of the International Grail Movement, Marian Ronan and Mary O’Brien contribute substantially to remedying this situation.Founded in the Netherlands in 1921, just after World War I, the Grail movement was focused, from the outset, on using laywomen’s extraordinary gifts to resolve the crises in which the world found itself. By 1961, the movement had spread to twenty other countries, including Brazil, Australia, the Philippines and nine African countries.Drawn from interviews done with Grail founders in many of these countries, Women of Vision highlights the relentless and often heroic work done by Grail women, founding and staffing hospitals and schools, supporting indigenous women and girls, preparing local women for church and Grail leadership, and in some cases, assuming governance roles in their own countries and at the United Nations. "The Grail founders whose voices are heard in this volume were and are women of great faith and enterprise. Their stories are invaluable to the history of the movement that inspired them.” —Alden V. Brown, author, The Grail Movement and American Cath...

To achieve this purpose, 75 prostate cancer patients of ages 63.5± 3.5 years and 33 apparently healthy individuals of ages 61± 3.8 years (control group) were enrolled. The distribution of patients revealed 60% (40) of rural areas, 44% (30) are smokers, 52% (39) having either heart diseases and/or hypertension, and 40% (30) of positive family history for the disease. The levels of MDA, GSH, GPX, and CAT were measured in sera of patients and control group. The statistical analysis exhibited significant elevation for MDA levels and significant decreases for GSH and GPX levels in the group of patients when compared with those of the control group. Patients from rural areas showed significant rise for MDA values when compared with those of urban areas. Variations of oxidative stress markers were found to be essentially related to prostate cancer and independent on smoking or other diseases in the studied patients. MDA levels were demonstrated to increase significantly in patients of positive family history when compared with those of negative family history of prostate cancer.

Depression in Neurologic Disorders Diagnosis and Management Edited by Andres M. Kanner, MD, Professor of Neurological Sciences and Psychiatry, Rush Medical College at Rush University, Chicago, IL, USA. Why should neurologists care about depression? It is one of the most frequent psychiatric comorbid conditions of neurologic disorders. Not only does it have a negative impact on the quality of patients, but it is also associated with a worse course of the neurologic disease. Yet, more often than not, neurologists have not been trained to recognize clinical depression in their patients. How should neurologists learn to care for depression? By reading Depression in Neurologic Disorders, the caring neurologist will gain a practical overview of: clinical manifestations of depression in the major neurologic disorders how to use screening instruments in clinical practice the identification of patients with an increased suicidal risk basic principles of the management of depressive disorders by the non-psychiatrist The major neurologic disorders covered in Depression in Neurologic Disorders include: Migraine Stroke Epilepsy Parkinson’s Disease Multiple Sclerosis Huntington’s Disease Dementia Traumatic Brain Injury The practical approach is enhanced with the use of case studies and ‘take-home pearls’ throughout. Depression is a common disorder that all neurologists face in their daily clinical practice. Depression in Neurologic Disorders provides the fundamentals of caring for patients who are suffering beyond their diagnosed neurologic disorder. Centering around a black social movement that W.E.B. DuBois started in 1905, this book is a case study that focuses on the Niagara Movement in order to analyze the patriarchy, sexism, and socially constructed gender ideology that formulates black social movements past and present. This book offers an in-depth study of the exclusion of black women from the Niagara Movement, focusing especially on how black womens exclusion shaped the Movement. I argue that by applying black feminist theory to the theoretical foundation of the Niagara Movement, it can be reconfigured to reflect the needs of the collective U.S. black community and thus serve as a template for present and future black social movements.

What's the Crack? When cracking is observed in a building the most common reaction is that it must be the foundations and that it must be serious. Many surveyors are nervous about diagnosing cracks. This is understandable, as the interpretation of cracks can be difficult to teach, with university courses providing little or no time for field experience. Yet by understanding one straightforward principle the majority of cracks can be diagnosed in just a few minutes. Linking this with a good knowledge of construction techniques, the factors that distort cracking patterns and the key features of common causes of cracking, almost all cracks can be diagnosed quickly and with confidence. Practical Guide to Diagnosing Structural Movement in Buildings provides a methodology by which cracks and movement in buildings can be diagnosed. Written in four parts, part one describes the key principles of movement and cracking. Parts two and three describe the main features of common forms of movement and the associated crack patterns. Part two covers causes other than ground or foundation movement and part three covers movement caused by ground or foundation problems. Part four briefly describes the techniques used to arrest further movement or repair damage cause by movement. Primarily intended for the relatively inexperienced surveyor or engineer and undergraduate students of surveying as a practical guide for use on site, it focuses on identification and diagnosis. It will not only help in correctly diagnosing the problem but it will also demonstrate a methodical approach to show and record how the diagnosis was reached – critical when giving advice to third parties. «What's the crack?» – In the vast majority of cases it's not foundation related and in most cases it's not indicating a serious defect.

Bachelor Thesis from the year 2017 in the subject Sociology - War and Peace, Military, grade: 1.3, University of Tubingen (Institute of Political Science), language: English, abstract: By 2018 the United Nations referred to the conflict as the "worst man-made humanitarian crisis of our time" and simply a "stupid war": overshadowed by the Syrian crisis, the civil war in Yemen was largely ignored by western media or simply referred to as a proxy-war between the Kingdom of Saudi Arabia and Iran. However, the essay shows that the situation on the ground witnesses a much greater complexity. A major civil war-party is the so-called "al-Houthi movement" from northern Yemen. Surprisingly, the movement has transitioned from a peaceful social movement through a violent movement against President Saleh's regime, to a movement supporting the uprisings during the "Arab Spring", holding national political power and seeking seats in a coalition-government. Taking into account their major role in the country's history, presence and possibly Yemen's future setup, this paper aims to go beyond the conventional explanations. It "deorientalizes" the movement and sheds light on the Houthis as a social movement with particular aims, resources and constraints that are subject to change.

People with AIDS often suffer from major physical and psychosocial distress due to their experience of serious life-threatening illness. The palliative care needs of AIDS patients at Tikur Anbessa hospital are very high and interrelated. Their palliative care needs are not addressed by the existing health care services due to limited access. Besides the available services are not holistic and they are not integrated with other services. Therefore promoting standard palliative care services for patients and their families is important to alleviate their distress and improve their quality of life. This book focuses on exploring the palliative care needs of AIDS patients from the perspectives of the AIDS patients and health care providers. The book came up with workable recommendations which could be used as an input in designing and implementing appropriate palliative care services in existing heath care system.

This book is the story of 'the SCM tradition' as it began and developed, as it suffered division, struggle and perhaps went astray, and as it has sought to rediscover its roots, as it faces the future in hope. "Robin Boyd shows just how much the ecumenical movement of the twentieth century owes to the SCM. So why did a movement that changed the course of church history decline in the late 1960s? Here is a well argued, penetrating and convincing answer which sheds light on today's complex ecumenical scene and has implications for the future direction of the ecumenical journey." Kate Scannell Death of the Good Doctor. Lessons from the Heart of the AIDS Epidemic

Kate Scannell abandoned her academic career in 1985 expecting to enter an "ordinary" medical practice in Northern California. Instead, the thirty-two-year-old physician found herself assigned to an Alameda county hospital's AIDS ward where much of the medicine she had studied over many difficult years was rendered irrelevant.Working with AIDS patients, nearly all of whom were dying, Scannell discovered the inadequacy of the "good doctor" who battles illness to keep patients alive regardless of their suffering. By embracing her patients’ unique needs and stories, Scannell reached an expanded understanding of her patients and of herself as a physician.Death of the Good Doctor richly chronicles the intimacy of Scannell’s relationships with her patients through whom the vast complexities of the AIDS epidemic are uniquely focused. It is through these beautiful, often difficult, and sometimes humorous portraits that the woman and the physician discover each other. From the Back Cover"This haunting memoir is an important addition to the canon of AIDS literature. Scannell writes beautifully and with an insight that escapes most physicians."—Abraham Verghese, author of My Own Country and Cutting for Stone“Kate Scannell is the rare doctor who has been transformed by her patients. Drawing on her own on background as well as her extended research study period during which she became a band member and was closely involved in its day-to-day affairs, the author, Dr Sylvia Bruinders, documents this centuries-old expressive practice of ushering in the joy of Christmas through music by way of a social history of the coloured communities. In doing so, she traces the slave origins of the Christmas Bands Movement, as well as how the oppressive and segregationist injustices of both colonialism and apartheid, together with the civil liberties afforded in the South African Constitution (1996) after the country became a democracy in 1994 have shaped the movement.

The first book focusing specifically on frequent and frequently disabling side effects involving the skin, hair and nails in cancer patients According to the World Health Organization, there are approximately thirty million people living with a diagnosis of cancer – the majority of whom will receive surgery, systemic therapy, and/or radiation, and who will suffer from dermatologic adverse events. Dermatologists and oncologists are only beginning to grapple with these events, which pose serious quality-of-life issues with so many patients, and will become more prevalent as survival rates improve, thanks in part to new cancer treatments and drug regimens. Concentrating on a topic that has only been briefly touched upon by other texts, this book offers a focused perspective on the clinical presentation, underlying pathophysiologic mechanisms, and management of skin, hair, and nail conditions for oncologists, dermatologists, and allied practitioners. Dermatologic Principles and Practice in Oncology: Conditions of the Skin, Hair, and Nails in Cancer Patients: • Covers in detail the dermatologic adverse events of oncologic therapies, clinical presentations, and treatment recommendations • Enables dermatologists and other practitioners to significantly improve the care of patients with cancer • Addresses the dermatologic adverse events of cancer therapies used globally, of which a large number are found in developing countries • Emphasizes prophylactic measures – based on treatments used and type of cancer – to prevent the appearance of adverse events • Provides built-in discussions on patient education for practical counseling during therapies • Offers rapid-reference sections on topical dermatology drugs The first book to present dermatologic conditions in cancer patients and survivors in a uniform and in-depth manner, Dermatologic Principles and Practice in Oncology is ideal for oncologists, oncology nurses, and dermatologists who wish to take better care of those with adverse skin, hair, and nail conditions.

The elderly represent the fastest growing segment of the population in developed countries, reflected in the patient population presenting to EDs and hospitals. These patients more often than not have greater co-morbidities, more complicated workups and utilize more laboratory and radiologic services. This text is designed to teach emergency physicians how best to care for this specific demographic of patients. It addresses physiologic changes, high-risk conditions, and atypical presentations associated with elderly patients in the ED that result in frequent misdiagnosis or delays in diagnosis. It instructs the readers how best to care for elderly patients in order to minimize morbidity and mortality, addressing some of the difficult psychosocial issues that confront health care providers that care for elderly patients, such as psychiatric disease and end-of-life care. The utility of this text is not limited to emergency physicians, but it should be useful to all health care providers involved in the treatment of elderly patients with acute medical or surgical conditions.

Robert Ogman Against the Nation. Anti-National Politics in Germany

Following the German reunification process in the 1990s, a new movement appeared in Germany. This movement rejected all forms of nationalism, including the desirability and legitimacy of national communities, borders, and the existence of the nation-state itself. Against the Nation covers the background of this movement—the rise of Neo-Nazism, racist violence, restriction in immigration policies, and growing state power—as well as its urge to organize society around other principles than nationality.By examining the campaigns and documents of the various anti-national tendencies in Germany during this period, Robert Ogman takes a fresh look at the question of nationalism and its relationship to Left politics.

Hepatitis B and C viruses (HBV & HCV) are hepatotropic viruses causing viral hepatitis, chronic liver disease and hepatocellular carcinoma. Hepatitis B and C virus infections are still major public health problems around the globe. Both of these viruses are transmitted mainly through the parenteral route and therefore a dual infection of these viruses can occur and even persists in the same patient. They are prevalent in different parts of the world including Ethiopia. In view of this, hospital based cross-sectional study was conducted in three public hospitals of Addis Ababa over a period of 7 months (Nov 2010- May 2011) on clinically diagnosed chronic liver disease patients. Serum samples from each volunteering patients was screened for the presence of HBsAg and anti-HCV Ab.

Jay Noricks Parts Psychology. A Trauma-Based, Self-State Therapy for Emotional Healing

This book adopts a novel, even revolutionary, approach to healing a wide range of psychological problems in therapy. The premise is that all of us have a number of multiple personalities within us who powerfully influence every aspect of our lives. By locating these internal parts and neutralizing the effects of the high-energy, often traumatic, experiences that created them, Parts Psychology demonstrates that patients can heal rapidly and completely from long-held emotional issues. The core of the book contains the healing narratives for 12 patients who, except for the problems that brought them to therapy, lead relatively normal lives. Several chapters describe the treatment process for such problems of emotional intimacy as lost love, low sexual desire, jealousy, and sexual swinging. Others describe issues of compulsion such as binge eating, porn addiction and bulimia. Several chapters detail success stories in the treatment of anger and rage, depression, grief and anxiety. Child abuse appears in the history of a number of patients. Each story begins with the first meeting with the therapist and concludes when the patient graduates from therapy.A first look at patients’ inner worlds might suggest to some the presence of multiple personality disorder (dissociative identity disorder). And many patients are shocked to find that they can have conversations with themselves. However, the case studies illustrate that having unconscious parts (subpersonalities), represented by a ra...

Michael Lienesch In the Beginning. Fundamentalism, the Scopes Trial, and the Making of the Antievolution Movement

The current controversy over teaching evolution in the public schools has grabbed front-page headlines and topped news broadcasts all across the United States. In the Beginning investigates the movement that has ignited debate in state legislatures and at school board meetings. Reaching back to the origins of antievolutionism in the 1920s, and continuing to the promotion of intelligent design today, Michael Lienesch skillfully analyzes one of the most formidable political movements of the twentieth century. Applying extensive original sources and social movement theory, Lienesch begins with fundamentalism, describing how early twentieth-century fundamentalists worked to form a collective identity, to develop their own institutions, and to turn evolution from an idea into an issue. He traces the emerging antievolution movement through the 1920s, examining debates over Darwinism that took place on college campuses and in state legislatures throughout the country. With fresh insights and analysis, Lienesch retells the story of the 1925 Scopes "monkey" trial and reinterprets its meaning. In tracking the movement from that time to today, he explores the rise of creation science in the 1960s, the alliance with the New Christian Right in the 1980s, and the development of the theory of intelligent design in our own time. He concludes by speculating on its place in the politics of the twenty-first century. In the Beginning is essential for understanding the past, present, and ...

Nowadays, the number of heart attack patients is increasing day by day. Though it is tough to save heart attack patients, we can increase the statistics of saving the lives of those patients as well as the lives of those whom the heart attack patients are responsible for. The main design of this project is to track heart attack patients suffering a heart attack during driving, send them medical help and stop the vehicle they are driving to ensure that the persons along them are spared from accidents. An eye blinking sensor is used to monitor the eye blinking rate and a spO2 sensor is used to check the pulse rate of the patient. Both are connected to a micro controller. If eye blinking stops, then the signal is sent to the controller to make an alarm through the buffer. If the spO2 sensor perceives a variation in the pulse rate or low oxygen content in the blood, which may result in heart failure, then the controller stops the motor of the vehicle. Then, a Tarang F4 transmitter is used to sent the vehicle number and the mobile phone number of the patient to the nearest medical station within 25 km for medical aid. The pulse rate monitored via LCD .The Tarang F4 receiver acquires the signal and passes through the controller, the number gets displayed on the LCD screen and an alarm is produced through a buzzer as soon the signal is received.Five topics are discussed in this project: detecting the patient BPM and the eye blinking status; transmitting via Tarang F4 in case of abno...

OccupyMedia! The Occupy Movement and Social Media in Crisis Capitalism

The Occupy movement has emerged in a historical crisis of global capitalism. It struggles for the reappropriation of the commodified commons. Communications are part of the commons of society. Yet contemporary social media are ridden by an antagonism between private corporate control (YouTube, Facebook, Twitter, etc.) and self-managed, commons-based activist media. In this work, Christian Fuchs analyses the contradictory dialectic of social media in the Occupy movement. Drawing on a political economy framework and interpretation of the results of the OccupyMedia! Survey, in which more than 400 Occupy activists reported on their social media use, OccupyMedia! The Occupy Movement and Social Media in Crisis Capitalism shows how activists confront the contradictions of capitalism and communication in the age of crisis and social media. The book discusses the contradiction between commercial and alternative social media and argues that the existence of a surveillance-industrial complex expressed in the PRISM system shows the urgent necessity to create social media beyond Facebook and Google.

Vincent Nyangaresi, Silvance Abeka, Solomon Ogara Statistical analysis of the GSM (Global System for Mobile Communications) mobility prediction models

Research Paper (postgraduate) from the year 2017 in the subject Computer Science - Miscellaneous, grade: -, , course: Information Technology Security & Audit, language: English, abstract: In the GSM environment, mobility prediction is concerned with envisaging of the mobile station's next movement. By accurately employing the predicted movement, the GSM network is capable of attaining enhanced resource allocation and reservations, better assignment of cells to location areas, more efficient paging, and call admission control. Numerous studies have been carried out in mobile station prediction and as such, many mobility predictions have been developed. The goal of this paper was to statistically analyze these mobility prediction models in order to understand their strengths and weaknesses. The results of this analysis indicated that the current mobility models base their predictions on movement patterns of users throughout physical space. However, they do not directly relate to movement throughout the network based on the cell presently hosting a mobile station. In addition, several base transceiver stations may overlap over a single physical location, a complication not captured by the current mobility prediction models. Therefore, there is need for a generic mobility prediction model that can depict movement of mobile users more realistically within the GSM network coverage areas. As such, this paper proposes the development of a mobility prediction model that is cap...

Molecular biology is a rapidly developing field of science. Advancements in molecular biology have helped various other fields including medicine and dentistry to go molecular. Since the introduction of molecular biology in orthodontics and the identification of genes associated with growth and development the focus has shifted towards a personalized treatment approach. The genes which have been linked to malocclusions and developmental disorders can serve as targets for an individual based treatment. Researchers have also been able to discover the molecules involved in orthodontic tooth movement, thus paving a pathway for modulation of the rate of tooth movement. The field of molecular orthodontics is in its nascent stage. Most of the studies involving gene therapies have been conducted in animal models. Large scale clinical trials are required to develop precise treatment modalities. There is a constant need for further research in this field so that the benefits of this approach can be delivered to the patients in an effective way.

The needs of cancer patients and their families are complex. Yet still more complex are the powerful social and cultural forces that shape the delivery of cancer care, and the way in which it experienced. Cancer Nursing: Care in Context addresses this by adopting a unique approach that situates cancer care in the context of society‘s attitudes to the disease, and the broader every-day needs of both patients and their carers. By combining evidenced-based information, a critical view of care and treatment, and ‘first-hand accounts’ of having cancer and caring for people with cancer, the book provides a new perspective how best to deliver the care that patients truly require. This second edition includes new chapters on prevention, diagnosis, symptoms, self-help and self-management, and living with cancer long-term. It also: • Addresses significant changes in cancer care, including expansion of nursing roles, the establishment of cancer networks and cancer collaborative projects • Considers NICE guidance on Supportive and Palliative care • Incorporates material on prevention and early detection • Includes evidence tables based on relevant literature Cancer Nursing: Care in Context is an invaluable resource for all those involved in the provision of cancer care and support to patients and their families.

We conclude that survivin does not induce considerable humoral responses in lung cancer. Potentially, however, strong anti-survivin antibody responses can be elicited during the follow-up of the patients, whose clinical significance remains to be elucidated. These findings, together with our previous data concerning survivin expression and the related cytolytic T cell responses in lung cancer, signify a high tolerogenic potential of this tumor-associated antigen.

Critical Small Schools. Beyond Privatization in New York City Urban Educational Reform

Critical Small Schools: Beyond Privatization in New York City Urban Educational Reform features themost current empirical research about the successes and challenges of the small schools movement and theimplications of such for urban public educational policy. Situated in a climate of hierarchical reform, manyof the principles of the original small schools movement-which are rooted in community participation, innovative pedagogies and assessment, and equity and social justice-have become obscured by aneducational agenda that emphasizes top-down mandates and standards-based reform. With the increasedpopularity and the rapid proliferation of small schools, the emphasis on 'size only' has resulted in abifurcation of the small schools movement; on one end are the small schools which have embraced thedemocratic, participatory, and self-governing nature of the original movement, while on the other end areschools that have simply reduced their size without rethinking school structures and practices. This book distinguishes the small schools featured and researched in this volume from schools that are simply small and labels them "critical smallschools." By documenting the practices that take place in various critical small schools in New York City, we show how these schools have narrowedthe achievement gap and increased graduation and college acceptance rates. Although smallness is an essential feature in the design of these schools, itis certainly not the only one and...
