This book was a thesis submitted in partial fulfillment of the requirements for the degree of Master of Science in Regional and Rural Development Planning at the Asian Institute of Technology, Thailand. The thesis on "Assessment of health services under the universal coverage scheme (UCS) for rural people in Thailand" was aimed at identifying strengths, weaknesses and needs as regards the scheme so as to provide recommendations to make it more beneficial for the rural people. The research was conducted in rural area of Surin province, in Northeastern Thailand. Main findings of this research showed that mistargeting of the UCS beneficiaries brought about benefit leakage; fee at the primary care unit (PCU) level was flexible; and health personnel had usually violated villagers' right to choose their registered PCU. Medical supplies and equipment accessible at a PCU were modern enough to provide primary care however their shortage and contamination were problems. Its disease prevention and health promotion budget was misused for other purposes. As for organization, there was close collaboration of medical and paramedical personnel within the studied primary care network.

This research is designed to analyse the effects of public spending of education and health on income distribution which examine the pre-expenditure and post-expenditure income distribution in Thailand. It follows the benefit incidence analysis (BIA) that is a method of computing the distribution of public expenditure across different demographic groups, such as women and men. The procedure involves allocating per unit public subsidies (for example, expenditure per student for the education sector) according to individual utilization rates of public services. This paper aims at examine who are the real beneficiaries from the government expenditure. The study uses the quantitative method where data are used of 2010. From the benefit incidence analysis on the public expenditure on education it seems that education system is pro-poor and health care system are not pro-poor rather pro-rich. This situation may create problems in healthcare services especially ineffective health plan resulting difficulties in maintaining health standards and resolving challenges in health. Studying on this conflict and interpretation of real health situation is vital for provision of sustainable healthcare service. Aim of this study to develop a social dialogue on current situation of health and evaluate its sustainability. Research was carried out using primary data collected from the community. Findings of primary data indicated inadequate facilities in the state hospitals. Seventy two percent of admitted patients were requested some drugs and laboratory test to be out sourced. It could be found every citizen spends at least less than 1000 rupees monthly for their health needs, even in low income levels in the community....

This article raises the problem of equity in the health system in Switzerland. Three dimensions of the concept of equity are taken into consideration: the inequality in the financing of the health system, the inequality in the distribution of the state of good health, and, finally, the iniquity in the access to health care. Some methodological developments are presented as well as the results. In conclusion we observe that the state of good health does not depend strongly on income but that it exists some iniquity in the access to health services from specialists and that the income inequality is increasing due to the financing of the health system.

The existence of regional inequalities in health care services is a common phenomenon at global level. Efforts are made to reduce the regional inequalities by promoting equitable access to primary health care (PHC) services and facilities. This study explores the inter-district variation in primary health care services and development in Balochistan. The study uses a set of twelve (12) PHC related indicators, out of them ten (10) are related to curative health care and two (2) indicators are selected covering the curative aspect of primary health care. By using 0-1 transformation value technique, the composite indexes are calculated for curative and preventive health care services. Districts are ranked as under developed, moderately developed, developed and less developed. There are found sharp inter-district variations in curative as well as preventive health care services in Balochistan. In order to promote the primary health care services in Balochistan the proposed recommendations include ensuring equitable provision of health professionals particularly in terms of female doctors, and decentralization of tertiary care services in order to promote primary health care services.

This book used the Upper East Region of Ghana as a case study to examine the contribution of the Catholic Church (a not-for-profit health care provider) to improving access to health care delivery in rural communities. The research used secondary and primary data sources which produced results indicating the immense health care delivery gap being filled by the Catholic Church in rural Ghana. It outlined several dimensions of health care access and identified public-private partnerships as a viable policy for governments to adopt for improved access to health care. This study aimed to analyze the impacts of public health policies in Brazil in the period 2000-2010, checking its collaboration on the improvement of public health, also investigating the ethical and efficiency of its management aspects. Therefore, was used the methodology of panel data in order to assess the variation in health spending between 2000 and 2010. The results showed that the variation in health spending in Brazil is positively correlated to the development of States, so that is still observed large differences in investment in public health policies among states, mainly in the North and Northeast regions of the country.

Nach Beantwortung dieser Fragen habe ich dargestellt, welche Faktoren zu der Entstehung der jetzigen Systeme geführt haben. So ist das Thema dieses Buches die vergleichende Analyse der politischen Entwicklung der zwei asiatischen Staaten Thailand und Indonesien. Meine Arbeit beginnt mit einem theoretischen Teil in dem das Gerüst für die Länderanalyse aufgebaut wird. By 2007, health centers in primary health care are public entities, autonomous, contracted by HII, financed according to package of services, self managed. Health centre is managed by Manager and economist that are responsible for financial and human resource management. HC are financed performance based according to a new formula: 85% of budget was given monthly for normal functioning of services, 10% was given monthly as performance based and 5% was called quality bonus, given quarterly to HC. Study mean to measure impact of financial mechanism of health centers in function of health staff performance, by components: Finances, performance and level of health staff knowledge about reform. Target of study are primary health care centers, family doctors and nurse employed in selected health centers. 684 health staffs are interviewed by 52 health centers randomly selected as part of the survey. It is found positive statistical correlation between bonus level and health staff knowledge about reform(r=0.218, p=0.023), and continues medical education (r=0.607. p<0.001).
