Robert Brewer The Politics and Process of Reorganisation. the NHS in England 1965-74

The tripartite structure of hospitals, general practitioners and local authority public health services which characterised the establishment of the National Health Service in England and Wales in 1948 tended to be regarded as the best available compromise at the time and as probably temporary in nature. To a considerable extent, it represented the outcome of a tussle between the important interest groups involved, with the medical profession largely emerging triumphant, local government retaining relatively low status public health services, and the voluntary hospitals losing everything. This research which is presented here, was undertaken almost contemporaneously with the events described, examines the process through which the first reorganisation of the National Health Service in England and Wales was undertaken between 1965 and 1974, viewed through the eyes of the principal players - the Ministry of Health/Department of Health and Social Security, the Treasury, the various branches of the medical profession and the local government bodies. The process is placed in the context of various ideas concerning the delivery of health and social care services and management in public administration which had emerged since 1948, together with the potential impact of local government reform which was then under consideration. The shifting balance between the demands of central vertical control for reasons of budgetary and policy control against the need for horizontal integration ...

М. С. Majo Income and health care utilization among the 50+ in Europe and the US

This study addresses the question how income affects health care utilization by the population aged 50 and over in the United States and a number of European countries with varying health care systems. The probabilities that individuals receive several medical services (visits to general practitioner, specialist, dentist, inpatient, or outpatient services) are analyzed separately using probit models. In addition to controls for income and demographic characteristics, controls for health status (both subjective and objective measures of health) are used. We analyze how the relationship between income and health care utilization varies across countries and relate these cross country differences to characteristics of the health care system, i. e., per capita total and public expenditure on health care, gate-keeping for specialist care, and copayments.

There are wide inconsistencies between, and even within, countries in how community-orientated care is defined and interpreted. The analysis presented in this book take as a starting point an evidence-based balanced care model in which services are provided in community settings close to the populations served, with hospital stays being reduced as far as possible, usually located in acute wards in general hospitals. The surprising conclusion from the research is that the same problems arise in all countries, regardless of resource status, and thus the recommendations of this book apply to mental health provision everywhere. This book reviews the implementation of community-orientated care using the balanced care model. It summarizes the steps, obstacles and mistakes that have been encountered in the implementation of community mental health care worldwide and presents guidelines on how to avoid them. It proposes realistic and achievable recommendations for the development and implementation of community-orientated mental health care over the next ten years. These guidelines will be of practical use to psychiatrists and other mental health and public health practitioners at all levels worldwide, including policy makers, commissioners, funders, non-governmental organisations, service users and carers. A core message of the book is that the mental health sector will more powerfully advocate for better services in future through strong and unified alliances, especially with powerful representation from consumer/service user and carer groups. Community-orientated care draws on a wide range of practitioners, providers, care and support systems (both professional and non-professional), though particular components may play a larger or lesser role in different settings depending on the local context and the available resources, especially trained staff. Research by a WPA task force has demonstrated that most of the challenges are common and global, but with local variations. The book is therefore relevant to psychiatrists and mental health workers in developed countries who are trying to deliver better health care on reduced budgets and for those in the developing economies who are in the position to modernise their mental health care. It provides clear, concise guidance on policy and practice decisions, learning from what has and has not worked in regions in the world. The book contains many tables documenting the evidence, supported by an essential reference list, and a Key Points summary for each chapter. Highly Commended in the Psychiatry section of the 2012 BMA Book Awards.

Written by the President and CEO of the Institute for Healthcare Improvement (IHI) and a leading health care journalist, this groundbreaking book examines how leading organizations in the United States are pursuing the Triple Aim—improving the individual experience of care, improving the health of populations, and reducing the per capita cost of care. Even with major steps forward – including the Affordable Care Act and the creation of the Center for Medicare and Medicaid Innovation – the national health care debate is too often poisoned by negativity. A quieter, more thoughtful, and vastly more constructive conversation continues among health care leaders and professionals throughout the country. Innovative solutions are being designed and implemented at the local level, and countless health care organizations are demonstrating breakthrough remedies to some of the toughest and most expensive challenges in health care. Pursuing the Triple Aim shares compelling stories that are emerging in locations ranging from Pittsburgh to Seattle, from Boston to Oakland, focused on topics including improving quality and lowering costs in primary care; setting challenging goals to control chronic disease with notable outcomes; leveraging employer buying power to improve quality, reduce waste, and drive down cost; paying for care under an innovative contract that compensates for quality rather than quantity; and much more. The authors describe these innovations in detail, and show the way toward a health care system for the nation that improves the experience and quality of care while at the same time controlling costs. As the Triple Aim moves from being largely an aspirational framework to something that communities all across the US can implement and learn from, its potential to become a touchstone for the work ahead has never been greater. Pursuing the Triple Aim lays out the vision, the interventions, and promising examples of success.

Emmanuel Boon, Albert Ahenkan Impact of deforestation on medicinal plants in Ghana

Research Paper (postgraduate) from the year 2008 in the subject Forestry / Forestry Economics, Vrije University Brussel (Human Ecology Department), 52 entries in the bibliography, language: English, abstract: The role of medicinal plants in traditional health care delivery in Ghana cannot be overemphasized. More than 250 indigenous trees and plants with healing properties have been scientifically catalogued in Ghana. Unfortunately, the very foundation upon which the medicinal plant species and the traditional health care system survive is threatened by deforestation. The rate of deforestation has increased by 50% over the last ten years, according to the UN Food and Agriculture Organization (FAO). The current area of intact forest is now estimated at between 10.9 and 11.8% of the original cover and 6.9% of the country's total area. Deforestation is changing the habitats of disease-carrying insects and creating conditions that may help to spread malaria, river blindness and other devastating illnesses. Moreover, since the majority of the rural poor in Ghana depends on traditional medicine for their health care needs, the present high rate of deforestation will have a detrimental effect on the heath care delivery system in the country. Important plant species will be lost to deforestation unless urgent measures are taken. This paper examines the impact of deforestation on medicinal plants in Ghana.

The global community clamors for American innovation and ideas. But no one wants our health care system.Americans spend trillions of dollars on health care only to generate some of the worst health outcomes in the world. Addressing health care’s problems with incremental solutions cannot generate the transformational change that’s desperately needed. Janis Powers offers a visionary alternative—the Dream Plan.Powers advocates for a system rooted in the values of the American Dream: personal accountability, longitudinal goal-setting, and community support. Her proposal requires a complete decentralization of the current payment system, ostensibly eliminating both health insurance and Medicare while dramatically altering Medicaid. Some of the thought-provoking points in this rigorously researched book include:• Why health insurance will soon be obsolete• Why a longitudinal perspective on health is critical to improving outcomes and saving money• Why the private sector, not the government, must drive health care innovation• Why all Americans must brace for rationing in health care spendingThe Dream Plan elevates the role of preventative care, creates a more market-based economy for health care goods and services, and shifts more accountability for outcomes to the patient. Powers offers an ambitious plan that serves as the first step in the transformation of the American health care system.Janis Powers is a health care strategy consultant who lives in Austin, Texas.

Panezai Sanaullah Inter-district Variation in Health Care Services in Balochistan

The existence of regional inequalities in health care services is a common phenomenon at global level. Efforts are made to reduce the regional inequalities by promoting equitable access to primary health care (PHC) services and facilities. This study explores the inter-district variation in primary health care services and development in Balochistan. The study uses a set of twelve (12) PHC related indicators, out of them ten (10) are related to curative health care and two (2) indicators are selected covering the curative aspect of primary health care. By using 0-1 transformation value technique, the composite indexes are calculated for curative and preventive health care services. Districts are ranked as under developed, moderately developed, developed and less developed. There are found sharp inter-district variations in curative as well as preventive health care services in Balochistan. In order to promote the primary health care services in Balochistan the proposed recommendations include ensuring equitable provision of health professionals particularly in terms of female doctors, and decentralization of tertiary care services in order to promote primary health care services.

Contemporary Issues in Mental Health Care in sub-Saharan Africa

Seventy percent of the global burden of mental disorders is located in low and middle income countries (LMIC),including sub-Saharan Africa (SSA). However, in Africa, only 0.62% of the national health budget is allocated to mental health compared to a global median of 2.8% and 5% in Europe. The government is the source of funding in 62%of patients with severe mental disorder in the World Health Organisation (WHO), Africa Region, the lowest of all the WHO regions, and lower compared to a global median of 79%. This is compounded by poor resources, with mental health outpatient facilities in WHO Africa Region being less that 10% of the global median. To address these problems, the WHO launched its Mental Health Action Gap Programme (mhGAP) in 2008, to scale-up mental health services in low and middle income countries (LMIC). The book is directed to all policy makers in sub-Saharan Africa to aid decision making about the urgent need for sustainable and relevant mental health care strategies, and the important areas that need priority. The book should be helpful to local and international researchers in formulating research questions relevant to the African continent and it will be of interest to medical practitioners and students in the region as adjunct to standard text books.

Mukasa Aziz Hawards The Underlying Dynamics of Health Care Systems in Developing Countries. Health Policy, Planning and the Impact of Social Economic Status (Ses) on Heal

The impact of health policy and planning are tremendous remedies through which the health care systems derive their primary potentials in the health promotion ventures and interventions. The programs designed in the health arena are tentatively equivocal and submissive to the implications influenced by the policy development criteria and much more open to the planning process. Every day, healthcare organizations are faced with crucial decisions about improving their systems of care and a lack of critical information to guide them. The research they need should be designed to help them provide better care to the patients in their organization, effectively and efficiently.Where does the Medicaid and the medical care strategies, as pursued in the United States of America, have their remedies through policy structure and together with the political culture associated to the system, or perhaps how possible is it that japan has the highest life expectancy co-efficient.The book is more of a fact file as results were generated by the health care research from suitable sectors and comparisons derived from the well-off health care states mainly from the OECD fraternity, as these bare differences due to policy even when their economic bases differ by small percentages. Bachelor Thesis from the year 2009 in the subject Medicine - Public Health, grade: 2, , language: English, abstract: This study examined the relationship between marriage, social support, and psychological health among impoverished, rural mothers. While research suggests marital status and social support are negatively correlated with depression, research investigating such relationships and effects on poor, rural mothers over time is scant. To examine the roles of marital status, social support, and time on depression, mean comparison and analysis of variance were run for depression levels across time, categories of partnership, categories of social support, and categories of change in marital status over time. While findings revealed that marital status had little effect on depression levels, social support appeared to be negatively correlated with depression. Furthermore, mothers who maintained marital status and reported high social support reported the lowest depression levels. Findings contribute to the limited body of research focusing on health in rural areas and yield valuable knowledge about the experience of psychological health among impoverished, rural mothers.

Mental health is an integral component of health that helps a person to realize his or her own relational abilities using the psychomotor, affective and cognitive domains. A high prevalence of mental disorders is seen in old age and predominant among these is depression. The community-based mental health studies have revealed that the point prevalence of depressive disorders among the geriatric population in India varies between 13 and 25 per cent. Another common mental health problem of the elderly is dementia. The total prevalence of dementia rises sharply above age 75. Individuals with dementia have reduced survival. The prevalence of cognitive impairment has been estimated between 11% (cognitive impairment- no dementia: ages 65-74) and 98% (memory impairment on at least one objective test). The financial burden including medications, care giving and institutional care; increases with the severity. This book explores the mental health status of the elderly Indian population. It also evaluates the National Mental Health Programme in India and identifies the scope of improvement in the mental health care delivery systems for the elderly.

Scientific Essay from the year 2017 in the subject Medicine - Alternative Medicine, , language: English, abstract: Integrative health care encompasses measures and activities that aim to achieve an increase of human health resources and the potential of maintaining and improving human health. The term describes the process of empowering individuals to increase their control over determinants of health and thus to positively impact their health. The focus is hereby not only on an individual's behavior, knowledge, and abilities, but also on social, economic, and environmental circumstances. From a holistic perspective, health is defined as physical, psychological, and social wellbeing that is impacted by individual, social, and societal backgrounds. Hence, health is not so much a condition or a goal but rather a resource of daily life. Due to demographic developments, we experience an increase of the population segment with chronic conditions while at the same time less people are paying into the social security system. Chronically ill people not only suffer from their primary disease but also the associated side effects, as in the case of co- and multi-morbidities. A growing shortage of qualified personnel in the service sector and the obscurity of health care structures complicate the access to qualitatively optimized health care. A transformed social framework, reflected in the abolishment of the intergenerational contract and the rise of single households, signifies an ...

It is derived from the authors’ extensive experience in articulating and implementing recovery-oriented practice and has been tested with roughly 3,000 providers who work in the field as well as with numerous post-graduate trainees in psychology, social work, nursing, and psychiatric rehabilitation. It has consistently received highly favorable evaluations from health care professionals as well as people in recovery from mental illness. This guide represents a new clinical approach to the planning and delivery of mental health care. It emerges from the mental health recovery movement, and has been developed in the process of the efforts to transform systems of care at the local, regional, and national levels to a recovery orientation. It will be an extremely useful tool for planning care within the context of current health care reform efforts and increasingly useful in the future, as systems of care become more person-centered. During the 1990s, reforms were introduced in the health care sector in many European countries with the common goal to make existing health care systems more cost efficient and gain greater control over how public resources were spent within them (Mossialos/Le Grand 1999). Kandappu Mylvaganam The Ethical Case for Clinical Ethics Committees

Modern health care increasingly faces ethical problems in relation to new technologies, costs and rationing and changing social values. An ageing population and an increase in the prevalence of chronic diseases have led to difficult choices in allocation of limited resources. There is greater public awareness of ethical issues through media reporting and a greater demand for public accountability of health care professionals. Health care professionals need not only strive to give the best possible clinical care, but also have to consider the ethical dimensions of the care they provide. To ensure high ethical standards in patient care, individual health care professionals and health care institutions need support and advice. The author argues that clinical ethics committees serve that purpose.

Some methodological developments are presented as well as the results. In conclusion we observe that the state of good health does not depend strongly on income but that it exists some iniquity in the access to health services from specialists and that the income inequality is increasing due to the financing of the health system.

In The Patient Revolution, author Krisa Tailor—a noted expert in health care innovation and management—explores, through the lens of design thinking, how information technology will take health care into the experience economy. In the experience economy, patients will shift to being empowered consumers who are active participants in their own care. Tailor explores this shift by creating a vision for a newly designed health care system that's focused on both sickness and wellness, and is driven by data and analytics. The new system seamlessly integrates health into our daily lives, and delivers care so uniquely personalized that no two people are provided identical treatments. Connected through data, everyone across the health care ecosystem, including clinicians, insurers, and researchers, will be able to meet individuals wherever they are in their health journey to reach the ultimate goal of keeping people healthy. The patient revolution has just begun and an exciting journey awaits us. Praise for the patient revolution «A full 50% of the US population has at least one chronic disease that requires ongoing monitoring and treatment. Our current health care system is woefully inadequate in providing these individuals with the treatment and support they need. This disparity can only be addressed through empowering patients to better care for themselves and giving providers better tools to care for their patients. Both of those solutions will require the development and application of novel technologies. In Krisa Tailor's book The Patient Revolution, a blueprint is articulated for how this could be achieved, culminating in a vision for a learning health system within 10 years.» —Ricky Bloomfield, MD, Director, Mobile Technology Strategy; Assistant Professor, Duke Medicine «In The Patient Revolution, Krisa Tailor astutely points out that 80% of health is impacted by factors outside of the health care system. Amazon unfortunately knows more about our patients than we do. The prescriptive analytics she describes will allow health care providers to use big data to optimize interventions at the level of the individual patient. The use of analytics will allow providers to improve quality, shape care coordination, and contain costs. Advanced analytics will lead to personalized care and ultimately empowered patients!» —Linda Butler, MD, Vice President of Medical Affairs/Chief Medical Officer/Chief Medical Information Officer, Rex Healthcare «The Patient Revolution provides a practical roadmap on how the industry can capture value by making health and care more personalized, anticipatory, and intuitive to patient needs.» —Ash Damle, CEO, Lumiata «Excellent read. For me, health care represents a unique economy—one focused on technology, but requiring a deep understanding of humanity. Ms. Tailor begins the exploration of how we provide care via the concepts of design thinking, asking how we might redesign care with an eye toward changing the experience. She does an excellent job deconstructing this from the patient experience. I look forward to a hopeful follow-up directed at changing the provider culture.» —Alan Pitt, MD, Chief Medical Officer, Avizia «Whether you're a health care provider looking to gain an understanding of the health care landscape, a health data scientist, or a seasoned business pro, you'll come away with a deeper, nuanced understanding of today's evolving health care system with this book. Krisa Tailor ties together—in a comprehensive, unique way—the worlds of health care administration, clinical practice, design thinking, and business strategy and innovation.» —Steven Chan, MD, MBA, University of California, Davis

Royer-Maddox-Herron Breaking Out of the Health Care Abyss. Transformational Tips for Agents of Change

It has become routine to see in various health care magazines, e-mails to leaders, and executive discussions on social media internet outlets, a listing of the ten top challenges that CEOs believe they are facing in the health care industry today. They often call this list the "what is keeping me up at night" list. Although the recognition of their current challenges is important for health system leadership teams and board members, just knowing them is not enough. To be successful in this extremely challenging and complex health care environment today, implementation of action plans is key. The recognition of this critical need to implement a successful road map to the future in order to address the disconnect between knowing and doing has driven the content of this book. Based on the less than positive performance, both clinically and financially, of many health care providers, it seems obvious that the implementation of successful action plans is sorely missing in many boards’ and leadership teams’ processes. The proof that such is the case lies in the fact that the yearly challenge list provided by a wide array of CEOs from small, medium as well as large complex health systems and across the broad health care sector has changed very little over the last decade. We hear and read over and over again about the lack of primary care providers, the declining inpatient volumes, the decrease in profitability, the variability in clinical outcomes, the lack of strong clinic...
